Previously known as Pueblo Medical, which served the community of Azusa for more than 10 years, Family Medicine Headquarters (FMHQ) is a medical practice under new ownership in Azusa that is ready to serve the neighborhood with compassionate family care.
The beauty and art of family medicine is that the team at Family Medicine Headquarters can provide care to patients from birth to their senior years. Our team is comfortable seeing a wide range of ages and ethnicities of people and our focus is to use evidence based medicine to diagnose and treat a wide range of acute and chronic conditions.
One of the practice motto's is that we want to treat the patient as if they are a member of our own family. Because of this, we really stand out as a practice because we are empathetic, friendly, respectful, and humane. It is extremely important to our staff and doctors to make patients feel welcomed at our practice from the moment they walk through our doors to the moment they leave their discussion with our doctors. There are dedicated personnel for specialist referrals and medication refills and also dedicated staff to ensure patient's receive their test results within a timely manner. The practice also provides same-day appointments and telehealth online visits.
